The polite form is ¿Como está?
The familiar or more casual form is ¿Cómo estás?

Do you understand? = ¿Entiende?

I don’t understand = No entiendo

I understand = Entiendo

I am sick = Estoy enfermo or Estoy enferma

I speak only a little Spanish = Solo hablo un poco de español
