The Resources for Resiliency (R4R) program was developed through a partnership between Urban Housing Solutions and Vanderbilt University School of Nursing in Nashville, Tennessee. A nurse-led intervention, the Resiliency Hub aims to break the perpetual cycle of chronic homelessness by providing comprehensive care and support within a subsidized housing facility. The Hub addresses physical health, mental health, and social determinants of health for marginalized individuals transitioning from homelessness and incarceration. Services offered by at the Hub include medical triage, COVID-19 testing, wound care, medication supply, resource navigation, housing stabilization, social support connection, and community engagement activities.
