To solve chronic back and SI joint pain it is critical to determine the UNDERLYING CAUSE through a correct diagnosis. There are imbalances that can often go overlooked that can set a person up for years, even a lifetime, of chronic pain and disability.

In this video, Dr. Brant Pedersen (DC) dives deep into one such cause that is many times hiding in plain sight, leg length discrepancy, aka leg length difference. He shares the story of a patient who came to him with hip and ankle pain that she thought was coming from SI joint dysfunction.

In this video Dr. Pedersen covers many aspects of leg length discrepancy starting with how much of a leg length discrepancy can cause a problem? He looks at the medical literature to help you understand what percentage of people have a leg length difference. Then he talks about how much of a leg length difference can cause pain.

Methods for determining leg length discrepancy are demonstrated. These tests include a standing visualization of the iliac crests. Another test involves lying on your back and looking at relative knee heights. He also discusses measuring between the ASIS and the medial malleolus with a tape measure. Lastly, Dr. Pedersen talks about the gold standing test which is taking full limb x-rays.

Dr. Pedersen talks about the three most common causes of structural leg length difference. These include trauma to a long bone growth center prior to skeletal maturity. Also, severe trauma to a bone that when it is repaired or healed is no longer even with its paired limb. Lastly, structural leg length difference can be caused secondary to a total hip joint replacement surgery or a total knee joint replacement surgery.

The treatment solutions for leg length discrepancy are discussed including using a heel lift on the short leg side and when a modification to the sole of a shoe is necessary.
