Diarrhoea or loose stool is a common clinical symptom. Acute and chronic diarrhoea are different and needs specialised care.

Acute diarrhoea: Less than 2 weeks. Mostly infections

Chronic diarrhea: More than 2 weeks

Causes
Poor digestion of food. Related to small bowel, liver and pancreas

Excessive bowel movements.

Any issue in digestion, will lead to chronic diarrhea.
1. Celiac disease : Allergy to gluten found in wheat. Villi are destroyed.

2. Parasitic infections
a. HIV
b. Post transplant
c. Low level antibodies

3. Lactose intolerance : Milk sugar. Not digested

4. Small bowel intestinal bacterial overgrowth: Glucose breath test



Excessive motility

1. IBS

2. Diabetes

Low enzyme : Chronic pancreatitis,
Low bile : Bile duct obstruction


Large bowel causes

Inflammatory bowel disease : Ulcerative colitis and Crohn’s disease
UC: Ulcers in large bowel
Blood in stool
Sigmoidoscopy

Usually no blood

CT scan and colonoscopy, capsule enodoscopy

Colon cancer: More secretion above the cancer
Villous adenoma : More secretions from the tumor
Diverticulitis

CMV colitis




Irritable bowel syndrome diarrhea
Food intolerance
